Please use the following form to report your monthly attendance averages.
In an on-going effort to collect accurate information to aid the District Sunday School Board to develop appropriate training and support, please provide the monthly average for classes that meet on Sunday as well as the average of small group attendees who are not included in the average Sunday attendance (small groups that include 30 minutes of church board approved study).
