You may need to update your Adobe Reader software (if it is not the latest version 7).  You can go here to start that download.

Downloadable Forms
You may open these forms with the free Adobe Acrobat Reader, available here, or with Microsoft Word.  Then you can fill in the form on your computer, print it, and mail it with your other reports.  The completed .pdf forms will not be saved on your computer; you will only print them.  If you use the MS Word documents you will be able to save a copy on your computer.
