When ready please click the link below to be taken to a website to download your church’s individualized document. The 2017 information for your church directory is provided, and we ask that you make any additions, corrections or changes before emailing it to the district office.
For all additions, corrections, and changes, please format in the same manner as shown in the document, and be as complete as possible with information provided.
NOTE: Give email address, telephone number including area code and complete address for each person.
**This document must be fully completed- including the requested contact information for each individual- not just the names of the individuals.**
